ATA Carnet General List Form
The green ATA Carnet General List Form is the reverse side of the ATA carnet cover. The columns to be completed by the applicant online are: Item No., Trade Description of goods and marks and numbers, if any, Number of Pieces, Weight or Volume, Value, Country of Origin. The General List form contains 30 lines of data. Continuation pages are available for an additional charge of $8.00 per page (eff. 7/1/2010).
